Just head to this page and you can get signed up. When I did this last year, it wouldn’t let me sign up my kids with their online system. I had to call their customer service department. So it might be the same for you.
But since one-way flights from SLC to LAX have been dropping down to $79 a bunch of times, you could use your $50 credit and get each round trip flight for $108!! (Only $54 each way!) No, if you have already done it, you can’t resign up for a new mileage account.

Hi, I had a question. We already booked a flight with them for September can we get that credit and apply it to our existing reservation? And just get a refund for that credit amount?

No, I believe their fine print says it is only for flights that haven’t already been booked.
