On the rebate form in the fine print, it states that you have to send another copy of the form to a different address. (bottom of fine print on rebate form)

AWESOME! I purchased the 5 quart one from Kohl’s that you posted… I didn’t know I can opt out of the magazine… it is only $12 for that one but IT IS $12!!! Thank you, I will be doing that! Also, I saved an additional 5% off the purchase through http://www.Perkstreet.com – February power perks(where you get 5% cash back) included Kohls.com so in the end I will have gotten the 5 quart for a total of $145!!! (Without a Kohl’s credit card)!!!
