Looks like they don’t send it in the mail to you. You have to print out the 20 page book and also print out the t-shirt thing then transfer it to your shirt your self. So as long as you have a printer you will be all set!

Me, too. I’ve seen this kind of form before and had trouble with it then. I started with my son’s b-day, then mine, then my older son, then my husband. It didn’t accept any of them.
