My rebate form is also slightly torn on the bottom right corner. (My Husband cut it out!) So I am missing part of the rebate address. Could somebody please give the the rest of the address? I’ve looked all over online and cannnot seem to find anywhere. It would greatly be appreciated! I have most of the address, i know it’s P.O. Box 472 Scottsdale Az 85252-0472.
