Here is the fine print of how this works per Amazon:
Step 1: Complete your purchase of any eligible Muppets title between November 4, 2011 and January 1, 2011.
Step 2: Once shipment of the disc occurs you will receive an e-mail within 48 hours containing your promotional code, click the link contained in the email to go to www.activaterewards.com/muppetsmovie and redeem your promotional code.
Step 3: On the www.activaterewards.com/muppetsmovie, website follow the instructions to enter your promotional code and other required information and print your certificate.

I’m wondering the same thing. I live in Logan and we don’t have the big chain theaters up here, so I want to know if I can for sure use it…. How do I find out?

In the fine print, it tells you where to redeem your movie tickets. Check that site and see if they have a list of movie theaters where the tickets can be used.
