Brienne- I would list your store as Amazon and your city as the city you live in, since technically that is where you were when you purchased the mixer. Regarding the missing packaging slip that is something you will have to contact Amazon directly regarding. Hopefully they can email you a copy of the packaging slip.

I had the same problem, they didn’t include a packing slip. I emailed them and they said I could print the invoice off. Does anyone know if that is okay for the rebate?

How do I opt out of the magazine? I do not see that option on the rebate form… and did anyone find out if the invoice is ok?
